Assistance With Self-Care Activities-Standard

Assisting with and/or supervising personal tasks of daily life, to develop skills of the participant, to live as autonomously as possible. These supports are generally conducted in the participant’s home and are to assist with the participant’s goals of living independently. The shifts also provide companionship and support to the client. What this essentially means is the support worker will attend the home and depending on the needs of the participant, may help the participant with showering and personal hygiene tasks, assist with preparation of meals, planning future meals, food shopping, assisting the participant with house cleaning, helping the participant schedule appointments, attend appointments, run errands & accomplish general domestic activities.

Assistance With Personal Domestic Activities

Assist participant to undertake and/or develop skills to maintain their home environment, where the participant owns their own home and/or has sole or substantial responsibility for its maintenance. Includes assisting participant to do basic house and yard work. In this category the support worker would attend the participant’s home and together the support worker and participant would complete domestic activities. These activities can include, however are not limited to, cleaning, meal planning and preparation, grocery shopping, mowing lawns, weeding & maintaining vegetable gardens. The support is aimed to assist the participant to live independently while maintaining a healthy and safe living environment and can be tailored to suit the participant’s unique living situation. ​

Assistance With Self-Care Activities – Complex

Assisting with, and/or supervising, personal tasks of daily life to develop skills of the participant, to live as autonomously as possible. This support group has been developed to be available to assist participant’s that have high support needs. These needs can be described as high physical support needs, behavioral support needs & intellectual support needs. Generally the support staff will require a higher level of qualification and there will be management plans in place to be able to best assist the participant. These supports can either be performed at the participant’s home or in the community, to help them to maintain involvement in school or other community based activities. The self-care activities that the participant receives support with are generally personal hygiene tasks including showering, toileting, shaving etc. If the participant lives independently they can also assist with general tasks of daily living including coordinating and attending appointments, house cleaning, meal preparation and food shopping.

Access Community, Social And Rec Activities

Provision of support to enable a participant to engage in a community, social and/or recreational activity, where a support worker with additional qualifications and experience relevant to the participant’s complex needs is required. In this support group the participant will receive support in the community to be able to maintain community involvement and maintain relationships within the community. The NDIS aims to protect the participants from becoming isolated or at risk of isolation because of the participant’s impairment. On these shifts the support worker would attend the participant’s home, help prepare the participant to access the community, then the support worker would travel with the participant to the activity and help the participant be actively involved in the activity. The support worker would then assist the participant home.

Group-Based Activities In The Community

Provision of support to enable a participant to engage in the community, social and/or recreational activities, where a support worker with additional qualifications and experience relevant to the participant’s complex needs is required. These supports are generally where the participant has a social group or group activity that they attend regularly and need assistance from a support worker to maintain the ability to attend. Depending on the level of required assistance from the participant, the support worker can either assist a participant at a one to one ratio or facilitate and coordinate a community activity with another participant, friend or family member. Generally the activity is planned by the participant and associates and the support worker is only there to help the participant when required.

Access Community, Social And Rec Activities

Provision of support to enable a participant to engage in a community, social and/or recreational activity. In this support group the participant receives the assistance of a support worker to be able to be involved in community activities that are of specific interest to the participant. The NDIS recognises that that each participant is an individual and has their own support requirements and interests. This support allows the participant to access and explore hobbies and interests with the assistance of a support worker. These shifts generally consist of the support worker picking up the participant from their home, taking them to the activity and helping them be actively involved in the activity. The participant is generally assisted at home and can request the support worker stay with the participant to provide further social interaction or assistance with self care activities.

Specialised Transport To School / Educational Facility / Employment / Community

Specialised transport to school / educational facility / employment / community. These shifts are mainly focused on supporting the participant to and from activities or appointments. The shift duration generally only covers the duration of the travel required. The staff member that is completing the shift is generally a trained support worker and can assist if there are behaviours of concern or special mobility assistance requirements.

Transport

Assistance with transport. These shifts are for participants who do not have the capacity to obtain and maintain a drivers licence and require a car as the mode of transport. These supports can assist the participant to either access the community for daily needs, including but not limited to accessing the supermarket, day services, community activities & social events.

House And/Or Yard Maintenance

Performing essential house and/or yard activities that the participant is not able to undertake. These activities can include and are not limited to; mowing, weeding, planting plants, watering, fixing fences and gates, installing letterboxes and general yard maintenance.

House Cleaning And Other Household Activities

Performing essential house cleaning activities that the participant is not able to undertake. These activities include but are not limited to; vacuuming, doing dishes, washing clothes, mopping floors, cleaning bathrooms & toilets. These are done separately to the participant, not involving them in the completion of these domestic duties.

Linen Service

Provision of clean linen to a participant unable to do their own laundry without assistance. These supports are not provided to the participant at the home of the participant. This service is structured so the company providing the supports would pick up the linen requiring cleaning and return the linen when clean.

Assistance With The Cost Of Preparation And Delivery Of Meals

Preparation and delivery of meals to a participant who is unable to do this themselves, and is not in receipt of other supports that would meet the same need. Food costs are not included. Cost varies with the number of meals prepared and deliveries required.

Assistance In A Shared Living Arrangement

Daily living support provided in a shared living arrangement. This rate is paid by the hour for one-to-one assistance, divided by the number of people supported. Depending on the level of care required for the participants, these shifts generally include assistance with personal hygiene tasks, assistance with domestic duties, and assisting with general duties including planning and coordinating appointments.
Supported Independent Living – Complex Assisting with and/or supervising tasks of daily life to develop the skills of the individuals with complex support needs to live as autonomously as possible. These supports generally require a more qualified staff member and are there to allow a person with high care requirements to maintain their independent living situation in an SDA (Specialised Disability Accommodation). The support worker is there to assist with personal hygiene tasks and domestic duties and coordinate and assist with general tasks of daily living when required. Supported Independent Living – Standard Assisting with and/or supervising tasks of daily life to develop the skills of the individuals with standard (or mixed) support needs, to live as autonomously as possible. These supports are conducted at the participants SDA and are to assist the participant to maintain their ability to live independently. They generally involve assistance with personal hygiene tasks, assistance with domestic duties and assistance with general tasks of daily living when required. Supported Independent Living – Lower Needs Assisting with and/or supervising tasks of daily life to develop the skills of the individuals with lower support needs to live as autonomously as possible. These supports are conducted at the participants SDA and are to assist the participant to maintain their ability to live independently. They generally involve assistance with personal hygiene tasks, assistance with domestic duties and assistance with general tasks of daily living when required.
Integrated support for self-care, accommodation, food & activities in a centre or group residence for short periods. Includes all expenses in a 24 hour period with no additional loading. May be used for up to 14 consecutive days, then weekly rates apply. This type of support is to allow a participant, when they are in a situation and they need emergency accommodation assistance, to be able to access alternative means of accommodation. These supports are generally only provided in emergency situations and are only used when there are no other accommodation options available.

Group Based Activities In The Community

Provision of support to enable a participant to engage in community, social and/or recreational activities. These supports are to encourage the participant to be involved in activities in the community. The support worker would generally attend the home of the participant, assist them to and from the activity and help them be actively involved in the group activity.

Extensive structural bathroom changes. For example, removal of a bath or hobless shower, electrical work, relocating plumbing, widening a doorway, waterproofing or changes to the fit-out such as toilet, tapware, basin, shower, rails paint and tiles.

Home Modifications – Bathroom Mod – Incl. Combining Bathroom, Toilet Or Laundry

Modifications to combine bathroom, toilet and laundry. Includes bath removal, hobless shower creation, plumbing relocation, electrical work, doorway changes, waterproofing and changes to bathroom fittings.

Home Modifications – Kitchen Adaption – No Structural Work

Modifications to a kitchen to enable a person with a disability to access and use it.

Home Modifications – Minor Miscellaneous Adjustments

Lower cost adaptions such as door adjustments, safety installations and bathroom fittings.

Fixed Vertical Lifting Platform – Home

Platform attached to stairs or vertical drop which a wheelchair or scooter rides on.

Free Standing Vertical Lifting Platform – Home

Non-enclosed lifting platform similar to a lift usually traversing smaller distances than an enclosed lift/ elevator.

Stair Lift With Seat

Seat and rails attached to stairs or incline for an ambulant person to ride upstairs on.

Stair Lift With Platform

Platform attached to rail or track to traverse person over stairs to upper or lower levels.

Drawings or plans which allow for agreements to be confirmed, e.g. modifications and for quotes to be obtained.

Project Management Of Home Modifications

Project manager or certifier independent of a builder, who can provide advice on the compliance of the plans with work undertaken and can coordinate quotes and payments.

Specialist Disability Accommodation (SDA)

SDA is an adjusted contribution to the cost of the physical building, including the land it is on. The quote will reflect the unique dwelling price based on the SDA enrolment process.

Funding to help you develop and strengthen new skills.

Exercise Physiology

Individual advice to a participant regarding exercise required due to the impact of their disability. These supports can assist a participant with strategies to help improve the overall health and fitness of the participant with exercises and activities tailored to the needs of the participant, conducted under the supervision of a qualified personal trainer.


Exercise Physiology In A Group

Advice to a participant regarding exercise required due to the impact of their disability, provided in a group setting.

These supports are to help improve the health and fitness of the participant as well as encourage social involvement. Some examples of these activities are Yoga, Zumba, Tai Chi etc.


Personal Training

Personal training provided to a participant due to the impact of their disability. These supports can assist a participant if the NDIS goals include improving health, fitness and overall health. The trainer can conduct these supports in the participant’s home or in the community.

Specialist Support Coordination

Generally delivered in a time-limited, outcomes-focused manner and by an appropriately qualified and experienced practitioner to meet the individual needs of the participant’s circumstances. Necessitated by specific high complex needs or high-level risks in a participant’s situation, to reduce complexity in the participant’s support environment in the context of broader systems of support, whilst also assisting the participant to connect with NDIS supports, negotiate solutions with multiple stakeholders and build capacity and resilience.

Self-Management Capacity Building

This support is to allow the participant to become a self managed NDIS participant. This means the participant would manage all service agreements, service bookings and invoices from service providers independently. This group allows the participant to receive training to be able to do this without the assistance of a plan management company or support coordinator.

Training In Planning And Plan Management

Capacity building and training in plan administration and management for participants unable to do this independently but who could do all or part of the task with training. This support is conducted by a plan manager, to train the participant on the payment and management of invoices required from the support provider.

Skills Development In A Group

Training for the participant in a group of 2 or more to increase their independence in daily personal activities. Some examples of the skills that can be developed are cooking, cleaning  & gardening.

Individual Skills Development And Training

Individual life skills development and training including public transport training and support, developing skills for community, social and recreational participation. These skills are generally specific to the participants needs. Some examples of the skills that can be developed under this category are shaving skills, washing clothes, cooking & managing financial obligations.

Individual Social Skills Development

Social skills development with an individual, for participation in community and social activities. These supports can be conducted by a support worker or mentor to increase the involvement and level of confidence to encourage the participation of the participant in social situations. Generally the support worker or mentor would attend social outings with the participant to assist to develop these skills.

Individual Skill Development And Training Including Public Transport Training

Individual training provided in the home for general life skills to increase independence. An example of some of the skills that can be developed are domestic activities, personal care tasks & catching public transport to community and social activities.

Training For Carers / Parents

Training for carers in matters related to caring for a person with disability. This support type is to assist the participant’s family with skills and knowledge to be able to assist the participant more effectively. Some examples of these skills are how to implement behavioural strategies from a psychologist and occupational therapist.
